CMS-121 is a quinolone derivative and an orally active acetyl-CoA carboxylase 1 (ACC1) inhibitor with potent neuroprotective, anti-inflammatory, antioxidative, and renoprotective properties.

Metabolomic analysis revealed that CMS-121 elevated plasma concentrations of short-chain acylcarnitines and butyrate metabolites, consistent with the effects of a ketogenic diet. This metabolic reprogramming towards ketogenic metabolism suggests that CMS-121 could be a promising treatment for weight loss and conditions like obesity and diabetes, where traditional ketogenic diets are difficult to maintain long-term.

Furthermore, CMS-121’s ability to modulate peripheral metabolism, in addition to its neuroprotective effects, positions it as a potential therapeutic agent for age-related neurodegenerative diseases.
